The course fees are due 14 days after the receipt of the invoice. For internal applicants: if a payment arrangement has been made with your respective institute/department, please indicate the amount and the account (Kostenstelle) below:*

Internal PMU applicants require a letter of approval from the head of their department or institute.

Cancellation Policy/ Course Withdrawal:
Cancellation sent via email at least 4 weeks before the course starts is free of charge. An administrative fee of Eur 100.00 will be charged for cancellations within 4 weeks from course begin. Course withdrawal after the course has started will be charged with the full module fee.

General terms and conditions of registration and participation:
By signing and submitting this form, you are officially registering bindingly for the course. Additional documents may be required for your registration. It is possible to book additional course modules. Please see the course program for further information.

COMPLETION:
Certificate of participation will be awarded after the successful completion of each module. Each module is awarded with 2 ECTS.

For PMU staff and PMU-Uniklinikum staff: provided that the full EMI course is successfully completed, two ECTS can be credited towards the didactic training requirements for habilitation.
